To the Supervisor of Registration of said County:
We, the undersigned, are registered voters in said State and County. As such, in accordance with Florida Statutes, 103.021 (3) we hereby petition you and the Florida Secretary fo State to place the name of the NATIONAL States Rights Party and its candidates for Preisdent and Vice President of the United States on the official ballot of the State of Florida to be used at the geneal election to be held on the first Tuesday after the first Monday in November, 1964, the same being the 3rd day of November, 1964. The candidates of the NATIONAL States Rights Party are John Kaspar of Nashville, Tennessee for President of the United States and J.B. Stoner of Mableton, Georgia for Vice President of the United States. We further petition you to ceritify this petition on to the Florida Secretary of State, as provided by 103.021 (3) of Florida Statutes.
NAME RESIDENCE (Street) POST OFFICE ADDRESS (City and State)
Note: All names of petitioners on each petition must be registered voters and live in the same County as stated above. Each petitioner should clearly sign his name and address as it appears on the records of the Supervisor of Registration. When completed, return this petition to the Party's County Chairman where you live, or the Florida State Chairman, or if you go ahead and file petitions, notify them and National Headquarters of number of names certified by Supervisor of Registration. The law requires that our Party pay 10 cents for each name submitted to the Supervisor of Registration in each County Chairman of N.S.R.P. State Executive Committee, Dewey M. Taft, P.U.Box 15065, Tampa 14, Florida Secretary fo said Committee, Daniel C. Johnson, P.O. Box 524, Vero Beach, Florida.
